Not sure I understand, either, but I'll try. :)
We have options for the "first line" of any document during export. It's essentially a "special paragraph", if you so wish.
You can also set the alignment of every part you export -- main text, title, first line and so on; just every paragraph style can have a different alignment, so you can have the first line be bold and centered etc.
However, there is no way to determine the "longest line" and have the page margins automatically adopt to that. If that's what you're after, you should export the "first line" centered, then go to word and adjust the page margins to your liking.
